2019 Moonlight Golf to Support Future Professionals

• 1. General Fee – $55.00
  This fee is for all participants. Includes three hours of Top Golf, and food and t-shirt. This events benefits all Health and Physical Education Future Professionals in the State of Arizona.
  
  Receipts for tax purposes will sent automatically or contact Matt Mixer (mixer@azhpe.org)
  
  Because this is a donation Benefiting Future Professionals, No Refunds will be given.
• 2. Group Fee – $300.00
  This discounted option is for one individual to purchase up to 6 registrations and guarantee that all six will be in the same Golfing Bay during the event.

• 3. Full-Time Student – $30.00
  Full-time college student is defined as taking at least 15 credit hours per semester. No Refunds for this event.
